Dear Patient:

It is our intention to serve you with the highest quality integrative medical services available. Unfortunately, we can no longer offer the type of complete medicine that we strive to practice under the requirements of private PPO insurance companies.

After September 25, 2007, the Institute for Progressive Medicine’s contract with Blue Shield will run out and we will no longer be a contracted provider. At that time, we will no longer be accepting Blue Shield for office visits and some services. However, you will be able to submit statements to your insurance for reimbursement for services that are covered within your contracted plan. Any reimbursement you receive will be considered out-of-network. We will be happy to provide the necessary paperwork for you to submit to your insurance company.

If you are a Blue Shield patient and wish to continue to receive treatment and services at the Institute for Progressive Medicine, payment will be due at the time of your visit. As a courtesy to all of our patients, we will continue to accept most private PPO insurance plans (including Blue Shield) for covered laboratory services performed in our office.

If you are a Medicare patient with Blue Shield PPO as a secondary payer, we will continue to bill your supplemental insurance as an out-of-network provider.

If you are unsure about the out-of-network coverage on your policy or if you have questions about your deductible or co-insurance for out of network services, we recommend that you call Blue Shield member services using the number on the back of your insurance card.
